Paper FOBT Kit Problems in Emergency Departments:

Ask any ED Doctor about FOBT kit use in his work (typically Hemoccult from Beckman Coulter), and you are likely to get surprising list of all the things that are wrong with use of existing paper kits and their separate developer bottles.

1.  There is still no better 1st level test for Gastro-Intestinal bleeding than the gFOBT POCT kit & that is why it is so universally used.  GI bleeding from any of dozens of conditions can NOT be ignored, as it can easily be fatal in a high % of cases if not detected early. Emergency Departments are thus on the spot to check for hidden GI bleeding before a patient is discharged or admitted.

2.  Many clinicians use FOBT kits, but there is supposed to be only one bottle for use in developing all the same lot coded kits. That means the bottle is usually somewhere else when needed.  The fecal contaminated kit goes to the bottle or the bottle, which may be contaminated, goes to the kit.  Neither choice is a good solution, and everyone knows it, as it wastes time, particularly when the bottle is "lost", meaning it is in someone's coat pocket.

3.  Emergency Depts by their nature see the most unkown infectious agents from patients coming in the door.  Using FOBT kits with reusable developer bottles & open paper kits with fecal matter requires exceptional care to avoid contamination in the ED, and then moving the contamination around the ED.  Everyone in the ED, Infection Control & other Departments knows existing paper FOBT kits are a less than optimal solution, but that is what is available.
